基层医院老年患者抗血小板药物应用分析

目的调查基层医院老年患者抗血小板药物使用情况，分析影响抗血小板药物使用率的相关因素，以了解基层医院老年患者抗血小板治疗的现状。方法采用现场问卷形式调查患者的基本资料和抗血小板药物使用情况。结果191例有抗血小板药物适应证老年患者，抗血小板药物总体使用率为70．16％（134／191），其中阿司匹林占67．91％（91／134）。抗血小板药物一级预防使用率63．83％（62／94），均使用阿司匹林。75岁以上组抗血小板药物服用率（79．26％）明显高于75岁以下组（63．30％），P〈0．05；冠心病组抗血小板药物服用率（79．75％）明显高于无冠心组（63．39％），P〈0．05。结论基层医院抗血小板药物使用现状与心血管疾病一级预防、二级预防指南推荐的均应服用抗血小板药物的要求存在明显的差距。

Application status of antiplatelet agents in elderly patients from community hospital

Objective To investigate the degree and determinants of antiplatelet agents use among elderly patients from community hospital. Methods The baseline characteristics and use of anti-platelet agents of these patients were collected by questionnaire. Results 191 elderly patients were involved,and all of them had the indication of using antiplatelet agents,but only 70.16% （ 134/191 ） patients were using antiplatelet agents. Among those patients using antiplatelet agents,67.91% （91/134） used Aspirin. The taking rate of antiplatelet agents in patients at age ≥ 75years （79.26%） was significant higher than at age 〈 75 year（63.30% ） （P 〈 0.05 ）. The taking rate of anti-platelet agents in patients with coronary heart disease（79.75% ）was significant higher than that in patients without coronary heart disease（63.39% ,P 〈 0.05）. Conclusion There is a gap between the application status of antiplatelet agents on community hospital and recommedation of the primary prevention or secondary prevention guidelines for cardiovascular diseases.
