锁定加压钢板治疗肱骨近端三、四部分骨折

目的:探讨应用肱骨近端骨折锁定加压接骨板(locking compressive plate,LCP)治疗肱骨近端三、四部分骨折的临床疗效.方法:应用LCP治疗肱骨近端骨折33例,根据Neer分型:三部分骨折23例,四部分骨折10例.采用经胸大肌三角肌入路.用Neer肩关节功能评分标准行术后患肩功能评分.结果:27例获得随访,平均随访时间10.6个月.3个月内骨折愈合19例,4个月愈合8例.按Neer肩关节功能评分标准:优14例,满意8例,优良率达81%.结论:LCP治疗肱骨近端骨折具有优良复位,固定牢靠,并发症少等优点,是一种较好的肱骨近端骨折的内固定物,值得临床推广.

Treatment of the three-or four-part humeral proximal fractures with locking compressive plate

Objective:To summarize the clinical effect of open reduction and internal fixation for the three-or four-part proximal humeral fractures with locking compressive plate(LCP).Methods:A total of 33 patients with complex humeral fractures were operated via deltoid-pectoral approach.According to the Neer classification,23 cases were three-part fracture,10 cases four-part fracture.Neer numerical rating system was employed to evaluate the postoperative function of the involved shoulders.Results:27 cases' follow-up was available with a mean of 10.6 months.19 cases got a union recovery within 3 months,8 cases within 4 months.According to the Neer numerical rating system,14 patients showed a excellent result,8 satisfactory,both together accounted for 81%.Conclusion:The treatment of proximal humeral fracture with LCP,which was approved to be a good reduction,excellent internal fixation and rare complications,could get a good clinical result and deserve clinical expanxion.
