肾上腺皮质激素对结节病的疗效及其对患者免疫功能的影响

目的　评价肾上腺皮质激素对结节病的疗效及对患者免疫功能的调节。方法　回顾性分析病例资料。结果　有病理根据的 2 0例结节病 ,男 7例 ,女 13例 ,年龄 14～ 6 3岁 ,临床症状除呼吸道症状外 ,有发热、关节肿痛、皮疹、视力异常、头痛、乏力等 ,胸部X线及CT示 19例双肺门肿大 ,1例单侧肺门肿大伴纵隔淋巴结肿大。用泼尼松 40～ 6 0mg/d治疗 ,连用 2～ 3个月后逐渐减量 ,T淋巴细胞转化率由治疗前 0 .5 4± 0 .18升高至 0 .6 9± 0 .2 3。SACE由 (6 9.8± 12 .6 )U/L下降至 (38.2± 12 .6 )U/L ,IgG由 (2 1.30± 4.18) g/L下降至 (16 .80± 4.2 3)g/L ,同时症状明显缓解。 结论　肾上腺皮质激素对结节病有特效 ,且对结节病患者的免疫功能有一定的调节作用。

Adrencortin's Curative Effect on Pulmonary Sarcoidsis and Effection on these Patients' Immune Function

Objective To evaluate adrencortin's curative effect on sarcoidsis and adjustment to these patients' immune function. Methods Reviewing investigation and analysis of clinic datium. Results In 20 cases of sarcoidsis, there were 7 male, 13 female. They were 14～63 years old. Clinic symptoms were fever, joint swell and pain, rash, sight abnormality, headache, fatigue, et al besides respiratory symptom. Chest X-ray and CT showed two lung gates of 19 cases swelled and one lung gate of 1 case swelled with mediastinum lymphaden swell. These patients were treated with prednisonum 40～60 mg/d. The dose were decreased gradually after having used for 2～3 months. T-cell transform rate increased from 0.54±0.18 to 0.69±0.23. SACE decreased from (698±126)U/L to (382±126)U/L and IgG from (213±418) to (168±423)g/L. At the same time, the symptoms were mitigated obviously. Conclusion Adrencortin is clearly effective on sarcoidsis and had moderate adjustment to patients' immune function.
