胸腰椎骨折矢状面弧度矫正与内固定关系的实验研究

摘    要:目的:实验验证经后路钉棒系统治疗胸腰椎骨折时,钛棒的折弯弧度与所固定节段矢状面弧度的等同关系,并讨论其影响因素。方法:取成年猪8具5~6个连续节段的颈椎、颈胸椎为实验标本,制成前中柱损伤模型,应用椎弓根螺钉系统复位固定,分析所固定钛棒的折弯角、所固定节段矫形后的cobb角,分析两角度是否存在显著差异及各种影响因素。结果:棒的折弯角度与钉棒系统所固定节段矫形后的cobb角两者比较差异无统计学意义(P>0.05)。结论:实验证明治疗胸腰椎骨折时,钛棒的折弯角度与所固定节段脊柱形成的cobb角度等同,能够较精确的预见术后恢复所固定节段的矢状面的弧度,为临床治疗提供依据。

The Empirical Study of the Correctional Angular on Sagittal Plane with the Internal Fixation in Curing Fractures of Thoraco-lumbar Vertebrate
Abstract:Objective:To validate that the bent angle of the stick is identical with the cobb angle of the reshaped and fixed segment on Sagittal Plane.Methods:The experiments were performed on the eight pieces of porcine C-spine or brustwirbel which had 6-7 bodies of the vertebra.Firstly,the standarded lateral projections were shot by X-ray and the cobb angles between the upper end-plate on the upper vertebralbody and the subtus end-plateon on the subtus vertebralbody were measured.Secondly,the central body of the vertebra was crashed into compressed or burst fracture.After reshaping with primary cobb angles the cobb angles of the reshaped segments were measured.And the cobb angles of the reshaped segments and the primary cob angles,were analyzed statistically.Results:The primal cobb angles were not statistically different from the cobb angles of reshaped vertebralbodies(P>0.05).Conclusion:the bent angle of the stick is identical with the cobb angle of the reshaped and fixed segment on sagittal plane.We can foresee the cobb angle of the reshaped spinal segment on sagittal plane according to the bent angle of the stick.
