内镜下氩离子凝固术治疗大肠多发息肉44例临床分析

目的:探讨内镜下氩离子凝固术(argon plasma coagulation,APC)治疗大肠多发息肉的临床疗效及安全性。方法:44例大肠多发息肉患者使用APC300型内镜专用氩气刀对广基扁平息肉进行内镜下APC灼除治疗。其中29例直径0.2~0.5 cm的广基扁平息肉仅行APC灼除;15例直径0.6~2.0 cm的亚蒂型、有蒂型及广基型大息肉行高频圈套电切,局部创面渗血者再行APC止血治疗。结果:44例患者均治愈。结论:APC治疗大肠多发息肉疗效佳,并发症极少,安全性好,且操作简便、快捷。对于多发性广基扁平小息肉,APC可作为首选治疗方法。

Endoscopic argon plasma coagulation for treatment of multiple polyps:report of 44 cases

Objective：To observe the effect and safety of argon plasma coagulation （APC） under endoscope in treatment of multiple polyps of the large intestine. Methods：The wide outsole and applanate polyp in 44 cases of multiple polyps of the large intestine was removed by APC under endoscope using APC 300 endoscope agron scalpel. The wide outsole and applanate polyp with a diameter of 0.2 - 0.5 cm was treated by APC only ; the pedunculated, semipedunculated as well as the wide and big outsole polyps with a diameter of 0. 6 - 2.0 cm were excised with high-frequency electrocision ;and after the therapy,these patients received APC for hemostasis. Results ： M1 the patients were cured. Conclnsions： Endoscopic APC is safe and effective for treatment of multiple polyps of the large intestine with the advantages of few complications and easy operation. It is the first choice for management of multiple sessile polyps of the large intestine.
