摘    要:目的:探讨胃癌术后病人早期肠内营养的临床效果及对机体免疫功能的影响. 方法:选择进展期胃癌根治性手术病人30例,随机分为两组:研究组(肠内营养组)15例,于术后24 h应用赫力广(Harminic-M)肠内营养,共实施7天;对照组(普通输液组)15例,术后给予常规输液治疗.两组病人分别于术前、术后第1天与第8天检测血常规、血糖、肝肾功能、电解质及免疫指标IgG、IgM、IgA、CD3 、CD4 、CD8 、CD4 /CD8 ,并观察有无严重并发症及胃肠功能恢复时间. 结果:术后第1天两组病人IgG、IgM、IgA、CD3 、CD4 、CD4 /CD8 比值均较术前有所下降,而术后第8天研究组IgG、IgM、IgA、CD3 、CD4 、CD8 、CD4 /CD8 等免疫指标迅速恢复,与对照组比较差异显著(P<0.05).两组病人在观察期间均无严重并发症,研究组病人胃肠功能恢复时间较对照组明显缩短(P<0.01). 结论:胃癌术后早期肠内营养支持可以恢复和改善病人的免疫功能,并有利于胃肠功能的恢复.

Abstract:Objectives: To Investigate the clinical effect of early enteral nutrition and immune function in patients with gastric cancer. Methods: Thirty patients were randomly divided into study group(EN group) and normal control group. The patients in EN group received Harmonic M early enteral nutrition from postoperative day 1 to postoperative day 8. The blood regular test, the blood sugar, the functions of liver and kidney, the blood electrolyte, IgG, IgM, IgA, CD3 ,CD4 ,CD8 ,CD4 /CD8 were detected on the day before the operation and the postoperative days 1 and 8 . The restoring time of Intestinal movement and complications were observed. Results: As compared with the day before the operation, all the levels of IgG, IgM, IgA, CD3 ,CD4 and CD4 /CD8 in two groups were decreased. However, the changes in the EN group was significantly different( P <0.05) from control group. In both EN group and control group there were no serious complications. The restoring time of intestinal movement was significantly decreased in the EN group( P <0.01) compared with control group after operation. Conclusions: Early enteral nutrition can improve immune function and promote the recovery of intestinal function in gastric cancer surgical patients.
