老年肱骨近端移位骨折的手术治疗

目的探讨老年肱骨近端移位骨折的外科治疗。方法对28例老年肱骨近端粉碎骨折采用切开复位,解剖型钢板内固定,术后早期康复治疗。结果28例随访3～24个月,平均15个月,采用Neer评分方法评定疗效,优良率为70%。2例四部分骨折者发生肱骨头坏死。结论对老年肱骨近端移位骨折采用切开复位,解剖型钢板内固定具有内固定牢靠、术后不需要外固定,早期功能锻炼的优点,有助于肩关节功能恢复。

Surgical treatment of proximal humeral fractures in the elderly

Objective To evaluate Surgical treatment of proximal humeral fractures in the elderly.Methods 28 patients who had been diagnosed as having comminuted and displaced proximal humeral fractures were operated and fixated using anatomical plate.Neer fracture classification system and rating system were used.Results All the case were follow-up 3-24 months,for an average of 15 months.According to Neer rating system,seventy percent of all reductions were excellent or good.Conclusion The results are satisfied if servere comminuted proximal humeral fracture are treated by open reduction and internal fixation by anatomical plate.The effect of fixation are attained well.It allows early functional treating.
