摘    要:目的研究膀胱移行细胞癌组织中血管内皮生长因子(VEGF)的表达特点,探讨其与间质微血管密度及肿瘤转移的关系。方法应用免疫组化S-P法,检测68例膀胱移行细胞癌、10例癌旁正常组织VEGF的表达,并在CD34染色切片上检测间质微血管密度(MVD)。结果膀胱癌组织中VEGF表达(45.6%)明显高于癌旁组织(20%),二者之间有显著性差异(P<0.01);VEGF阳性组MVD均值(47.63±14.81)高于VEGF阴性组(28.54±13.93),VEGF表达与MVD呈正相关(P<0.01)。此外,膀胱癌VEGF的表达与组织学分级、临床分期、淋巴结转移密切相关(P<0.01),而与患者年龄及肿瘤大小无关(P>0.05)。结论VEGF促进膀胱癌间质血管生成,促进肿瘤的浸润和转移,有可能成为判断膀胱癌生物学行为和预后的重要指标。

Expression of VEGF and its correlation with tumor interstitial microvascular density and tumor metastasis in bladder transitional cell carcinoma
Abstract:Objective To study the expression of vascular endothelial cell growth factor(VEGF) and its correlation with interstitial microvascular density(MVD) and tumor metastasis in transitional cell carcinoma of bladder(TCC). Methods The expression of VEGF and MVD were detected in 68 cases of bladder transitional cell carcinoma and 10 cases of corresponding non-tumor normal tissue by immunohistochemical technique. Results In comparison with corresponding non-tumor normal tissue (20.0%)-the expression of VEGF in bladder carcinoma (45.6%) was significantly higher (P<0.01). MVD was much higher in the group of VEGF positive expression than that in the group of VEGF negative expression- there was a positive correlation between VEGF and MVD(P<0.01). In addition-the expression of VEGF was correlated with histological grade and clinical stages and lymph node metastasis (P<0.01)- but there was no relationship with age and tumor size(P>05). Conclusions VEGF may promote the angiogenesis and play an important role in tumor invasion and metastasis. VEGF may be a important indicator to determine biological behavior and prognosis of bladder carcinoma.
