正常妊娠及子痫前期患者外周血中白介素-12、白介素-18的检测意义

目的 检测非孕妇女、正常妊娠孕妇及子痫前期患者外周血中白介素-12(IL-12)和白介素-18(IL-18)的水平,探讨Th1/Th2细胞因子在妊娠高血压疾病发病中的作用.方法 孕20周后的子痫前期组34例(轻度子痫前期15例,重度子痫前期19例),正常妊娠组32例,非孕对照组16例.取外周血进行流式细胞仪检测,选取CD28表达量最高者的淋巴细胞作为试验对象.采用ELISA法测定IL-12、IL-18,流式细胞仪测定Th1/Th2细胞比.结果 正常妊娠组、非孕组及子痫前期组IL-12比较均有显著性差异(P＜0.001).轻度子痫前期组、重度子痫前期组与正常妊娠组IL-18比较均无显著性差异(P＞0.05).正常妊娠组与子痫前期组IL-18/IL-12比较有显著性差异(P＜0.001).IL-18/IL-12浓度比与Th1/Th2细胞比的相关性比较,呈显著负相关(P＜0.01).结论 正常妊娠的IL-18/IL-12浓度比显著高于子痫前期患者,与Th1/Th2细胞比呈反向变化.

The ratio of interleukin (IL)-12 and IL-18 secreted by peripheral blood mononuclear cells in normal pregnant and pre-eclamptic patients

Objective To investigate the ratio of interleukin (IL)-18 to IL-12 secreted by peripheral blood mononuclear cells in normal pregnant and pre-eclamptic patients. Methods IL-18 and IL-12 secreted by non-stimulated peripheral blood mononuclear cells (PBMC) were measured in 32 healthy pregnant women, 15 mild pre-eclamptic patients and 19 severe pre-eclamptic patients. Sixteen non-pregnant women were controls. IL-18 and IL-12 level were examined by ELISA. Th1/Th2 ratios in PBMC were determined by flow cytometry. Results The ratios of IL-18 to IL-12 were significantly higher in healthy pregnant women than non-pregnant women. These ratios were significantly lower in severe pre-eclamptic cases than in normal pregnancy subjects, while these ratios in mild pre-eclampsia were similar to those in normal pregnancy. Th1/Th2 ratios were negatively correlated with the ratios of IL-18/IL-12. Conclusions The ratios of IL-18 to IL-12 are significantly higher in healthy pregnant women than pre-eclamptic women and Th1/Th2 ratio is negatively correlated with the ratio of IL-18/IL-12.
