摘    要:对95例胸部肿瘤患者经微创小切口行手术治疗。其中68例为右胸切口,27例左胞切口;切口长度为6—14cm,平均为9.8cm;手术时间29—130分钟,平均为48.5分钟;平均失血100ml。5例配合电视胸腔镜进行操作。本组无死亡。认为对于心肺功能不良的周围型肺癌,通过小切口行肿瘤切除术加术后综合治疗可以取得根治性疗效;对于周围型肺良性病变、小的纵隔肿瘤(<6cm)及食管平滑肌瘤,也可应用小切口手术;经小切口可行肺叶切除及纵隔淋巴结清扫术;经小切口行肺内微小结节切除可达明确诊断和根治的作用。

Abstract:To study the surgical treatment of thoracic tumor by small incision with minimal trauma, 95 cases were observed, 68 cases had incision on right thorax and 27 cases on left. The length of incision ranged from 6cm to14 cm(9.8cm on average), the time scale of operation from 29 minutes to 130 minutes (48.5 minutes on average). The average amount of hemorrage was 100ml, 5 cases were operated with the help of thoracoscope. There was no operative mortality. To the patients with peripheral lung cancer showing cardiac or pulmory dysfuction, radical curative effect can be reached by lump excision with small incision plus postoperative combining therapy, lump excision with small incision also can be indicated to peripheral benign tumor of lung, to small mediastic tumor (< 6cm ) and to esophageal leiomyoma. Small incision can be indicated to lobar pneumonectomy and mediastic lymph adenectomy. Excision by small incision can be indicated to intra-pulmonary small node to get clear diagnosis and radical cure.
