非脱垂子宫腹腔镜辅助阴式子宫切除术及阴式子宫切除术的疗效对比分析

目的 探讨非脱垂子宫腹腔镜辅助阴式子宫切除术（LAVH）及阴式子宫切除术（TVH）的临床疗效.方法 将2006年2月-2012年8月间在我院行TVH手术的46例患者作为阴式组,以同期行LAVH的45例患者为腹腔镜组,以同期行常规子宫切除术的42例患者为常规组.观察并比较三组组手术时间、术中出血量、术后肛门排气时间、术后住院时间、手术费用及并发症发生率.结果 常规组的手术时间明显长于腹腔镜组,而腹腔镜组平均手术时间明显长于阴式组,术中出血量则明显少于阴式组,手术费用明显高于阴式组,而常规组术中出血量和手术费用要明显高于阴式组和腹腔镜组,比较差异均有统计学意义（P＜0.05）;而三组术后肛门排气时间、术后住院时间及并发症发生率比较差异无统计学意义（P＞0.05）.结论 与常规组的子宫切除术相比,LAVH及TVH在手术时间、出血量及手术费用方面均具有优越性,LAVH出血量少,但手术时间较长,费用高,TVH手术时间短,费用较低,但出血量高.两组各具有优缺点,以供临床患者所选择.

Efficacy analysis of non prolapsed uterus laparoscopic assisted Vaginal hysterectomy and vaginal hysterectomy

Objective To investigate the non prolapsed uterus laparoscopic assisted vaginal hysterectomy （LAVH） and vaginal hys- terectomy （TVH） clinical curative effect. Methods The inter-February 2006 to August 2012 in 46 patients in our hospital TVH as vaginal surgery group to 45 patients for the same period in line LAVH laparoscopic group to the same period in routine hysterecto- my 42 patients for the conventional group. Observed and compared three groups of operative time, blood loss, postoperative anal exhaust time, postoperative hospital stay, surgical expense and complication rates.Results The average operation time of laparo- scopic group was significantly longer than the TVH group, and the amount of intraoperative bleeding was significantly less than TVH group, operation cost is obviously higher than that of female group, there was statistically significant difference （P〈0.05）; and the anus exhaust time, postoperative hospitalization after operation in the two groups after the occurrence rate had no significant difference in time and complications （P〉0.05）. Conclusion Compared with the conventional group hysterectomy, LAVH and TVH in operative time, blood loss and surgical costs are advantageous, LAVH less bleeding, but the longer operative time, high cost, TVH shorter operative time, less expensive However, the high amount of bleeding. Groups each having advantages and disadvan- tages for clinical patient selection.
