重叠综合征30例临床特征分析

[目的]探讨重叠综合征（OS）患者的临床特征及无创机械通气的疗效.[方法]对30例OS患者记录其体重指数、睡眠呼吸紊乱指数、第一秒用力呼气容积占预计值百分比、第一秒用力呼气容积占预计值百分比/用力肺活量等临床指标.其中25例接受无创通气治疗3个月后复查相关临床指标.[结果]所选患者上述指标明显异常.而无创机械通气可以明显改善患者病情.[结论]OS较单一慢性阻塞性肺疾病或阻塞性睡眠呼吸暂停低通气综合征相比其病情更严重,无创机械通气,尤其BiPAP可能成为OS最有效的治疗方法.

Clinical Characteristics of 30 Patients with OSAHS-COPD Overlap Syndrome

[Objective] To explore the clinical characteristics of overlap syndrome（OS） and the effect of noninvasive mechanical ventilation. [Methods] Thirty patients with obstructive sleep apnea-chronic obstructive pulmo nary disease（OSAHS-COPD） OS were included in this study. Body mass index（BMI）, apnea hypopnea index （AHI）, forced expiratory volume in one seeond（FEV1）, FEV1/foreed vital capacity（FVC） and so on were measured and analyzed. Twenty five patients were treated with noninvasive mechanical ventilation for three months. The results were analyzed before and after treatment. [Results] All above parameters in selected patients were significantly abnormal. Noninvasive mechanical ventilation could improve their conditions. [Conclusion] The patients with OS experienced more severity conditions than simple COPD patients or OSAHS patients. Noninvasive mechanical ventilation, especially Bi-level positive airway pressure（BiPAP） ventilation may be the most effective treatment.
