摘    要:目的 通过对机械通气并发下呼吸道感染(VAP)病原菌及耐药情况分析,为合理使用抗生素提供依据.方法 对我院ICU 2001年1月~2005年11月VAP 107例(156株)及耐药情况进行回顾性分析.结果 60岁以上VAP患者占本组资料67.3%(156株),痛原菌G-菌71.8%,其中大肠埃希氏27.6%,铜绿假单孢12.2%,亚胺培南,氨曲南对G-细菌耐药率最低.G 细菌以葡萄球菌属为主,占10.9%,万古霉素耐药率为0,真菌占10.9%.结论 VAP G-是主要致病菌,针对耐药率上升,改变抗生素干预策略至关重要.

Abstract:Objective To analyze the pathogenic bacteria in VAP and conditions of drug resistance so as to supply rational basis for antibiotic administration.Methods A retrospective investigation was conducted to the VAP cases admitted in our department between January 2001 and November 2005 and their drug administration.The total number of cases was 107(156 strains).Results Among the VAP cases investigated,those aged over 60 accounted for 67.3%.Pathogenic bacteria G-were 71.8% of the total 156 strains,of which Escherichia was 27.6% and pseudomonas 12.2% and imipenem aetreonam had the lowest drug-resistance.As for G+ bacteria,the major species was staphylococcus,which took 10.9% and had a low drug-resistance of 0% to vancomycin.Fungus accounted for 10.9% of the bacterial formation.Conclusion G-bacteria take the major part as the pathogenic bacteria of VAP.To counter the tendency of constant rising of drug-resistance,the most important measure is to change the strategy of antibiotics administration.
