口腔治疗防治鼻咽癌放疗后张口困难的临床分析

目的探讨口腔基础治疗对鼻咽癌患者放疗后张口困难的防治效果．方法对108例根治性放疗后出现张口困难的鼻咽癌患者按放疗前是否进行过口腔基础治疗分为A、B两组，以门齿距为观察张口困难的指标，对两组的门齿距进行测量；在进行功能锻炼及口腔基础治疗1个月后、半年后再进行门齿距的测量．结果A组患者未进行功能锻炼前85％为Ⅰ级张口困难，15％为Ⅱ级，程度较轻；B组患者52．9％为Ⅰ级张口困难，39．7％为Ⅱ级，7．4％为Ⅲ级；经过功能锻炼和口腔基础治疗1个月、半年后B组门齿距比A组增幅大．结论积极的口腔治疗和一些功能锻炼对于防止和减轻张口困难的发生是积极和有效的．

Oral Treatment for Prevention of Trismus in Nasopharngeal Carcinoma Patients after Radiotherapy

Objective To analyze the role of oral treatment for prevention of trismus in nasopharyngeal carcinoma （NPC） patients after radiotherapy. Methods From January 2005 to August 2006, 108 NPC patients treated with radical radiotherapppy were analyzed retrospectively. The patients who completed the oral treatment were brought into A group, the others was brought into B group. It was evaluated the temporomadibular joint lesion by SOMA＇ s criterion. The distance between two dens ineivus medialis of the patients was measured at first. Then it was measured at 1 month and 6 month after completed oral treatment and function training. Results A group, with grade Ⅰ, 34 cases （85%）; grade Ⅱ, 6 cases （15%）; B group, with grade Ⅰ, 36 cases （52.9%） , grade Ⅱ , 27 cases （39.7%） , grade Ⅲ , 5 cases （7.4%）. After function training and oral treatment, B group is much improved. Conclusion Function training and oral treatment are helpful to decrease the occurrence of trismus in NPC patients after radiotherapy.
