抗CCP抗体和RF联合检测在类风湿关节炎诊断中的临床价值

目的 研究抗环瓜氨酸肽（CCP）抗体和类风湿因子（RF）检测在类风湿关节炎（RA）诊断中的临床价值.方法 采用酶联免疫吸附试验（ELISA）、速率散射比浊法分别检测RA组87例、非RA组80例和健康对照组80例患者的血清抗CCP抗体和RF.结果 RA组血清RF和抗CCP抗体浓度分别为（72.56±22.36）U/ml和（152.69±28.55）U/ml,阳性率分别为80.45％和75.86％,均显著高于非RA组和健康对照组（P＜0.05）.抗CCP抗体对RA诊断的敏感性和特异性分别为75.86％和97.50％,RF对RA诊断的敏感性和特异性分别为80.45％和86.87％.在RA患者中抗CCP抗体与RF阳性率比较差异无统计学意义（x2=0.58,P＞0.05）.两种检测方法间无相关性（r=0.053）.结论 联合检测抗CCP抗体、RF可以互为补充,有助于类风湿关节炎的早期诊断和预测病情进展.

Clinical value of combined detection of serum anti-cyclic citrullinated peptide antibodyand rheumatoid factor in diagnosis of rheumatoid arthritis

Objective To study the diagnostic value of combined measurement of serum anti-CCP antibody and Rheumatoid factor in patients with rheumatoid arthritis （RA）.Methods Enzyme linked immunosorbent assay （ELISA）,rate nephelometry were used to detect anti—CCP antibodies and rheumatoid factor （RF）in patients with 87 RA,80 non— RA other rheumatic cases and 80 health persons group.Results The absolute concentration and positive rate of serum in RA group were 72.56±22.36 U/ml and 80.45％ for RF,152,69±28.55 U/ml and 75.86％ for anti—CCP antibodies,respectively,which were significantly higher than those in non-RA patients and health persons （P＜0.05）.The sensitivity and specificity of anti—CCP antibodies were 75.86％ and 97.50％ in RA group.The sensitivity and specificity of RF were 80.45％ and 86.87％ in RA group.Anti-CCP antibody and RF positive rate in RA group was no statistical significance （x2 =0.58,P＞0.05）.There was no correlation between two kinds of antibodies detection（r=0.053）.Conclusion Combined detection of serum anti-CCP antibodies and RF can complement each other,be helpful for early diagnosis and prediction of progression of rheumatoid arthritis.
