蛛网膜下腔出血患者外周血平均血小板体积的变化

摘    要:目的研究蛛网膜下腔出血患者外周血血小板平均体积(mean platelet volume,MPV)、计数的动态变化,探讨其可能的临床意义。方法 2011年1月-2012年12月在扬州市第一人民医院同期住院的蛛网膜下腔出血、急性脑梗死患者,蛛网膜下腔出血组25例,急性脑梗死组45例,老年对照组40例。患者于入院后第2天早晨7:00肘静脉抽血3 ml,查MPV、血小板计数(platelet,Plt)、血小板分布宽度(platelet distribution width,PDW)。计量资料采用t检验,P<0.05为差异有统计学意义。结果蛛网膜下腔出血组、急性脑梗死组、老年对照组MPV[(10.21±3.15)、(13.44±1.39)、(11.70±1.70)f/L]、Plt[(148.54±39.48)×109/L、(100.66±52.51)×109/L、(131.00±45.50)×109/L]比较差异均有统计学意义(均P<0.05),蛛网膜下腔出血组MPV、Plt发病后2、3周[(11.94±2.18)、(12.00±1.98)f/L、(132.09±40.75)×109/L、(125.54±35.08)×109/L]与发病1 d[(10.21±3.15)f/L、(148.54±39.48)×109/L]比较差异均有统计学意义(均P<0.05)。结论蛛网膜下腔出血后血小板计数和体积的变化可能与急性应激状态有关。

Abstract:Objective To study the changes of mean platdet volume (MPV) and platelet count (Plt) in patients with sub- arachnoid hemorrhage(SAH) and its clinical significance. Methods Twenty - five cases of SAH and45 cases of acute cerebral infarction(ACI) receiving treatment in the first people~ hospital of Yangzhou from January 2011 to December 2012 were selected, as well as 40 cases of elderly controls. Blood (3m1) was collected from cubital vein in all the subjects and measured for MPV, Pit and PDW at 7:00 am on the flint day after admission. Measurement data were processed with t - test, and the result of P 〈0.05 was considered to be statistically significant. Results MVP was (10.21± 3.15) f/L, ( 13.44 ±1.39) f/L and ( 11.70 ± 1.70) f/L respectively in SAH group, ACI group and control group. Pit was ( 148.54 ±39.48)×10^9/L, (I00. 66 ±52.51 ×10^9/L and (131.00 ±45.50)×10^9/L] respectively in SAH group, ACI group and control group. Significant difference was found between the three groups in MPV and Pit ( P 〈 O. 05 ). In SAH group, MPV and Plt were( 1 1.94 ±2.18 ) and ( 132.09 ± 40.75) ×10^9/L respectively 2 weeks after SAH onset, and ( 12.00±1. 98 ) f/L and (125.54±35.08) ×10^9/L respectively 3 weeks after SAH onset, which was significantly different from those on day 1 after SAH onset. Conclusions The changes of Pit and MPV may be related to the stress after SAH.
