不同体质量早产儿早期合理营养与体质量变化关系探讨

目的探讨不同体质量早产儿早期合理营养与体质量变化关系。方法随机选取2011年1月至2012年1月广东医学院附属南山医院新生儿重症监护室(NICU)收治的184例胎龄在28～37周的早产儿作为研究对象,按体质量分为两组:<2000 g组和≥2000 g组,比较两组早产儿早期的肠外营养及胃肠营养对体质量的影响。结果两组早产儿在出生后第2周肠外热量及蛋白质摄入有明显差异(P<0.05),且两组早产儿在开奶日龄、体质量开始增长日龄、回到出生体质量日龄方面有明显差异(P<0.05)。结论早产儿早期热量及蛋白质的摄入对出生后体质量的变化有明显影响,对早产儿,尤其是出生时体质量较低的早产儿,应及时给予合理的营养,以满足其生长发育的需要。

Discussion on the Relationship of Early Rational Nutrition and Body Mass Changes in Premature Infants with Different Birth Body Mass

Objective To study the relationship between early rational nutrition and weight change in premature infants with different birth body mass.Methods A total of 184 cases of premature infants(gestational age:28 to 37 weeks)treated in neonatal intensive care unit(NICU)of Nanshan Hospital Affiliated to Guangdong Medical College between Jan.2011 and Jan.2012 were enrolled into this study,which were divided into two groups,<2000g group and ≥2000g group.The effects of parenteral nutrition and enteral feeding on the body mass of the two groups were analyzed.Results There was statistically significant difference of the intake of energy and protein in the second week between the two groups(P<0.05).There were significant differences of initiation time of breast feeding,the initiation time of gaining body mass and returning time to birth body mass(P<0.05).Conclusion Early intake of calories and protein of preterm infants significantly influences the body mass changes after birth.Preterm infants especially low birth body mass infants should be provided nutritions to meet with the need of development of premature infants.
