胸腔闭式引流治疗胸腔积液482例疗效观察

目的探讨胸腔闭式引流治疗胸腔积液的疗效。方法将定边县人民医院2005年1月至2012年3月收治的482胸腔积液患者作为观察组,采用胸腔闭式引流治疗,另选择同期在该院治疗的360例胸腔积液患者作为对照组,采用胸腔穿刺治疗,观察比较两种引流方法的临床疗效。结果观察组总有效率为96.89%,对照组总有效率为87.50%,差异有统计学意义(P<0.05);两组治疗后肺功能效果优于治疗前,治疗前后比较差异有统计学意义,观察组治疗后肺功能各指标较对照组改变明显,且无一例并发症,积液消失时间及抽取积液量明显少于对照组,两组比较差异有统计学意义(P<0.05)。结论胸腔闭式引流具有创伤小,操作简单,并发症少等优点,可明显缩短治疗时间并提高治疗效果,是治疗胸膜恶性肿瘤、结核性胸膜炎、低蛋白血症等引起胸腔积液的有效方法,可在临床广泛应用。

The Clinical Observation of the Thoracic Closed Drainage in the Treatment for Pleural Effusion in 482 Cases

Objective To observe the clinical effect of the thoracic closed drainage in the treatment of pleural effusion.Methods 482 patients with pleural effusion admitted by our hospital from January 2005 to March 2012,who were treated with the thoracic closed drainage,were selected as the observation group.At the same time 360 patients who were treated with thoracentesis treatment,were selected as the control group.The effect of the two different kinds of draining methods was observed and compared.Results Total effective rate of the observation group was 96.89%of the control group was 87.50%,there were significant differences between the two groups(P<0.05).The lung function of the two groups after treatment were better than before.There were significant difference between before and after the treatment.Each index of the lung function in the observation group changed obviously than the control group after the treatment,and no complication was observed,the effusion disappearance time and extraction of effusion were less than that of the control group with statistically signficant difference(P<0.05).Conclusion The thoracic closed drainage has the advantages of small trauma,simple operation,and less complications.It can reduce the treatment time and improve the therapeutic effect,which is an effective treatment for pleural effusion caused by pleural malignant tumor,tuberculosis pleurisy and hypoalbuminemiaetc.,worthy of extensive application in clinical.
