成批特重烧冲复合伤员心理应激与早期干预

目的探讨成批特重烧冲复合伤员心理应激特征与早期干预方法。方法总结和分析一批16例特重烧伤伴冲击伤伤员的临床、心理特点及早期心理干预资料。结果几乎所有伤员在伤后早期均出现不同程度的心理应激反应,反应形式和反应过程略有不同,经早期进行干预此批烧创伤员心理异常得到明显改善。结论本组伤员在特重烧冲复合伤的基础上,早期出现了不同程度的心理应激反应,以焦虑和忧郁为主要表现。治疗上通过分析评估重度烧创伤后早期心理应激反应特点,认真制定心理干预方案,从伤后初期开展预防性治疗入手,通过与伤员建立有效沟通途径,帮助伤员正确的认知,同时辅以药物、放松及音乐等治疗,并结合临床治疗对心理干预疗效进行持续的综合评定,取得较好效果。

Psychological Stress and Early Psychological Intervention of a Batch of Patients with Extensive Compound injuries of burn and Blast

objective To explore characteristics of psychological stress and methods of early psychological intervention in a batch of the patients with extensive compound injuries of burn and blast. Method The clinical and early psychological intervention data in 16 patients with extensive compound injuries of burn and blast were analysed retrospectively. Results Nearly all the patients presented different degree psychological stress in the early stage after the injuries and the response form and process were slightly different from each other.The patients' psychological abnormality could be improved after early psychological intervention. Conclusions Anxiety and melancholy were main psychological phenomenon of early psychological stress in the patients with compound injuries. Treatment methods included making the intervention schemes based on analysing and evaluating the characteristics of early psychological stress prophylactic treatment early after the injuries effectively communicating with patient, helping patients to gain correct cognition and taking necessary medicines relaxation and listening to music The curative effect of psychological intervention on the psychological stress should be continuously evaluated in combination with clinical treatments.The satisfactory effect can be got by the above-mentioned treatment.
