侵袭性纤维瘤20例临床分析

[目的]探讨侵袭性纤维瘤的临床特点及外科治疗原则.[方法]回顾分析1987年1月～2003年12月间收治的20例侵袭性纤维瘤患者的临床病理资料.行局部广泛切除术者13例,姑息性手术6例,1例放弃治疗,术后行辅助放疗2例,1例放弃治疗.[结果]全组病例5年复发率30.0%,手术切缘阳性者5年复发率83.3%,手术切缘阴性者5年复发率7.7%.全组无术后死亡病例.[结论]侵袭性纤维瘤具有侵袭性生长及复发率高等特点;局部广泛切除肿瘤,保证切缘阴性为该肿瘤外科手术原则.

A Clinical Study on 20 Cases with Aggressive Fibromatosis

[Purpose] To investigate the clinical feature of aggressive fibromatosis and the principle of surgical resection. [Methods] Clinicopathologic data of 20 cases with aggressive fibromatosis from Jan. 1987 to Dec. 2003 were analysed. Of the 20 patients, 13 cases underwent wide local excision; 6 cases, palliative surgery and 1 case, gave up treatment. There are 2 cases radiotherapy in addition to surgery [Results] The overall local recurrence rate was 30.0%, while the recurrence rate in the cases with positive margine was 83.3% and the recurrence rate in the cases with negative margine was 7.7%. No death was found following operation. [Conchlsion]Aggressive fibromatosis has characteristic of aggressive growth and high recurrence rate. Wide local resection with negative marlines may be the principle of surgieal resection of this tumor.
