纤维支气管镜对成年患者下呼吸道异物的临床诊治

摘    要:目的探讨纤支镜诊治成年患者下呼吸道异物的临床价值。方法回顾分析我院2001年4月~2011年4月间140例成年患者下呼吸道异物病例的相关资料,其中80例采用纤支镜手术,60例采用支气管镜手术。结果本组纤支镜一次性治愈率明显高于支气管镜一次性治愈率,异物多位于右支气管。均成功钳取异物,122例异物位于右支气管,18例位于左支气管。术中79例直接窥见异物并直接取出;61例表面覆盖分泌物,其中21例肉芽组织增生明显,仅51例一次性取出。均无严重并发症。结论纤支镜是诊治成年患者下呼吸道异物的有效方法。

Clinical value of fiberoptic bronchoscopy for diagnosis and treatment of lower respiratory tract foreign body in adult patients

Abstract:Objective To investigate the clinical value of fiberoptic bronchoscopy for the diagnosis and treatment of lower respiratory tract foreign body in adult patients.Methods All of 140 adult patients with lower respiratory tract foreign body in our hospital from April 2001 to April 2011 were analyzed retrospectively,of whom 80 received fiberoptic bronchoscopic surgery and 60 received bronchoscopic surgery.Results The one-time cure rate of fiberoptic bronchoscopy was significantly higher than that of bronchoscopy.Foreign bodies mainly located in the right bronchus.All the foreign bodies were successfully removed from the patients,of which 122 were located in the right bronchus and 18 located in the left bronchus.During the operation,foreign bodies of 79 patients were directly glimpsed and removed;foreign bodies of 61 patients were covered with secretion,of whom 21 patients had obvious granulation tissue proliferation and only 51 patients had successful onetime removal.None of the patients had severe complications.Conclusion Fiberoptic bronchoscopy is an effective method for the diagnosis and treatment of lower respiratory tract foreign body in adult patients.
