右美托咪啶减轻股骨颈骨折患者麻醉时疼痛强度

目的：评价右美托咪啶用于下肢骨折患者行椎管内麻醉时的镇静镇痛作用。方法：拟在椎管内麻醉下股骨颈骨折复位内固定的患者60例,年龄50岁～70岁,体重45kg～80kg,汪五整ASAⅠ～Ⅱ随机分为三组（n=20）,对照组（C组）,芬太尼组（F组）和右美托咪啶组（D组）,分别在麻醉开始前15min给予生理盐水15mL,芬太尼0.01ug/kg和右美托咪啶0.5ug/kg,输注时间10min。观察并记录麻醉开始前（T0）、麻醉摆体位时（T1）、椎管内麻醉穿刺时（T2）、给药后4h（T3）、24h（T4）VAS评分和Ramsay镇静评分,以及操作时患者满意度。结果：VAS评分,T0、T3、T4时三组间无差别,T1、T2时D组和F组两组间无差别且都低于C组;Ramsay镇静评分,T0、T3、T4时三组间无差别,T1、T2时C组和F组两组间无差别且都低于D组;操作时患者满意度,D组和F组两组间无差别且都高于C组。结论：右美托咪啶能够减轻股骨颈骨折患者行椎管内麻醉时疼痛程度且有良好的镇静效果,能够提高患者满意度。

THE DEXMEDETOMIDINE PYRIDINE REDUCE THE FEMORAL NECK FRACTURE PATIENTS WITH NARCOTIC PAIN INTENSITY

Objective:Assessment of right dexmedetomidine for lower extremity fractures sedative and analgesic effect in patients with spinal anesthesia.Methods:Intended to spinal anesthesia,femoral neck fracture reduction and internal fixation in patients with 60 cases,50 to 70 years of age,body weight 45 ～ 80kg,ASA I-II were randomly divided into three groups(n = 20),the control group(C group),Fenfentanyl group(group F) and right dexmedetomidine group(D),respectively,during anesthesia before the start of 15 minutes were given saline 15mL,of fentanyl 0.01ug/kg and right dexmedetomidine 0.5ug/kg,loseNote time of 10 minutes.Before the start of the observation and record of anesthesia(T0),anesthesia pendulum position(T1),spinal epidural anesthesia(T2),4 hours(T3) after administration of 24 hours(T4),VAS score and Ramsay sedation score,as well as operating when patient satisfaction.Results:No difference between the VAS score,T0,T3,T4,three groups,T1,T2,no difference in Group D and Group F between the two groups and were lower than in group C;Ramsay sedation score,T0,T3,T4 among the three groupsgroup C and group F between the two groups when the difference,T1,T2,non-discriminatory and are lower than in group D;operation patient satisfaction,no difference in group D and group F between the two groups and were higher than C group.Conclusion:Right dexmedetomidine can reduce the femoral neck fracture degree of pain in patients with spinal anesthesia and the sedative effect,and can improve patient satisfaction.
