右美托咪定应用于ICU机械通气患者的镇静效果观察

目的探讨右美托咪定镇静在重症医学科（ICU）机械通气患者中的临床疗效。方法2013年4—11月收治的40例机械通气患者，按照随机数字表法分为右美托咪定组和咪达唑仑组各20例，分别给予右美托米定和咪达唑仑镇静。比较两组的镇静起效时间、停药后苏醒时间、机械通气时间及血流动力学指标。结果两组镇静起效时间差异无统计学意义。右美托咪定组停药后苏醒时间及机械通气时间短于味达唑仑组，心率及平均动脉压也低于咪达唑仑组（P〈0．05）。结论右美托咪定用于ICU机械通气患者镇静安全有效，且能缩短机械通气时间。

The sedative efficacy of dexmedetomodine for mechanically ventilated patients in ICU

Objective To investigate the sedative effect of dexmedetomodine for mechanically ventilated patients in ICU. Methods Forty mechanically ventilated patients in ICU from Apr. to Nov. 2013 were randomly divided into dexmedetomodine and midazolam groups,20 in each group. Patients in the two groups were sedated with dexmedetomodine and midazolam, respectively. The sedation onset time, awakening time, length of mechanical ventilation and hemodynamic parameters were compared between the two groups. Results There was no significant difference in gender proportion, age, acute physiology and chronic health evaluation II （ A- PACHE II） score and sedation onset time between the two groups. The awakening time and the length of mechanical ventilation in the dexmedetomodine group were significantly shorter than in the midazolam group （ P 〈 0. 05 ）. The levels of heart rate and mean artery pressure in the dexmedetomodine group were significantly lower than that in the midazolam group （ P 〈 0. 05 ）. Conclusion Dexmedetomodine is safe and effective for mechanically ventilated patients in ICU, and can shorten the length of mechanical ventilation as well.
