注射疗法结合关节松动术治疗肱骨近端骨折后肩关节活动障碍的临床效果

目的观察注射疗法结合关节松动术治疗肱骨近端骨折后肩关节活动障碍临床效果。方法将157例肱骨近端骨折后肩关节活动障碍患者随机分为治疗组79例和对照组78例,治疗组在关节松动技术的基础上应用注射疗法治疗肩关节活动障碍,对照组仅采用关节松动技术治疗。治疗前和治疗1个疗程后（3周）,分别评测肩关节疼痛程度和肩关节活动度。结果治疗1个疗程后,治疗组79例患者均无疼痛,肩关节恢复至正常61例、优14例、良4例和差0例,治疗组的总有效率（100.00%）和痊愈率（77.22%）均明显高于对照组,差异有统计学意义（P〈0.05）。结论注射疗法结合关节松动术治疗肱骨近端骨折后肩关节活动障碍,可以有效减轻或消除疼痛、改善肩关节活动范围,疗效显著。

Observation of injection therapy combined with joint mobilization on shoulder joint movement disability after proximal humeral fractures

Objective To observe the clinical effects of injection therapy combined with joint mobilization on shoulder joint movement disability after proximal humeral fractures. Methods 157 cases of shoulder joint movement disability after proximal humeral fractures were randomly divided into the treatment group （n=79） and the control group （n=78）. The treatment group received injection therapy combined with joint mobilization,the control group only for shoulder joint mobilization.Before and after one course treatment （3 weeks）,pain and ROM of shoulder were assessed,respectively. Results After one course treatment,all cases of the treatment group had no pain complain and 61 cases shoulder joint function recovery to normal,14 cases to excellent,4cases to better and no cases to bad.Total efficiency （100.00%） and cure rate （77.22%） of the treatment group had significant difference compared with the control group （P〈0.05）. Conclu-sion Injection therapy combined with joint mobilization to treat shoulder joint movement disability after proximal humeral fractures has significant effects,which can effectively reduce or eliminate pain and improve range of motion of shoulder.
