经皮穿刺椎间盘激光汽化减压治疗椎动脉型颈椎病

目的总结经皮穿刺椎间盘激光汽化减压术治疗椎动脉型颈椎病的疗效。方法 2004年8月至2008年9月间在我院住院的椎动脉型颈椎病患者共92例,其中男40例,女52例;年龄41～65岁,平均48岁。术前均经颈椎X线、颈椎MRI和椎动脉CT血管造影影像学检查诊断为椎动脉型颈椎病,在C型臂X线机或数字减影血管造影引导下,局麻后用16 G穿刺针从右侧穿刺,进入病变椎间盘中央稍偏后部,然后置入光纤,以自动脉冲式激光烧灼汽化,每个椎间盘使用激光能量500～1 000 J,分别于术后第1天、6个月、1年,按照Nagashima的标准评价疗效。结果平均随访18个月,按照Nagashima标准评分,术后第1天总体优良率为86.95%,术后6个月总体优良率为82.61%,术后1年总体优良率为80.43%。结论经皮穿刺椎间盘激光汽化减压术治疗椎动脉型颈椎病疗效确切,是一种安全、有效、简单、创伤小的治疗方法。
