血压晨峰与大动脉硬化程度的关 系

目的 探讨血压晨峰（MBPS）与大动脉硬化程度的关系.方法 选择高血压患者124例（高血压组,其中有血压晨峰56例,无血压晨峰68例）与健康体检者40例（对照组）.对所有受试者采用全自动动脉硬化测定仪测定其肱踝动脉脉搏波传导速度（baPWV）值,采用彩色超声诊断仪检测其颈动脉中膜厚度（IMT）和颈动脉斑块,并对3组测量值进行比较.结果 高血压组无任有无血压晨峰,其baPWV、IMT值及斑块检出率均显著高于对照组（P＜0.05）,而有血压晨峰的高血压患者其baPWV、IMT值及斑块检出率均显著高于无血压晨峰的高血压患者（P＜0.05）.结论 血压晨峰是影响大动脉硬化的重要因素,有血压晨峰的高血压患者更容易发生IMT增厚及粥样斑块的形成,其靶器官损害的危险性更高.

Relationship between Morning Blood Pressure Surge and Arterial Stiffness

Objective To investigate the relationship between morning blood pressure surge （MBPS） and arterial stiffness. Methods A total of 124 hypertensive patients were divided into MBPS group （56 cases）and non-MBPS group （68 cases）. In addition, 40 healthy subjects were selected as control group. Braehial-ankle pulse wave velocity （baPWV）was measured using automatic arterioscle- rosis analyzer. Carotid atheromatous plaques and carotid intima-media thickness （IMT） were determined using ultrasonic diagnostic instrument. The measured values were compared among the three groups. Results Compared with non-MBPS group, BaPWV, IMT and plaque detection rate significantly decreased in control group, but obviously increased in MBPS group （all P〈0.05）. Conclusion MBPS is an important factor influencing arterial stiffness. It can induce IMT thickening and atheromatous plaque formation and increase the risk of target organ damage.
