超声乳化白内障吸除联合人工晶体植入术治疗闭角型青光眼的疗效

目的探讨超声乳化白内障吸除联合人工晶体植入术治疗闭角型青光眼的临床疗效。方法将68例闭角型青光眼患者随机分为2组:对照组34例采用复合式小梁切除术治疗;观察组34例采用超声乳化白内障吸除联合人工晶体植入术治疗。比较2组患者术前、术后3个月的最佳矫正视力、眼内压以及手术并发症的发生情况。结果 2组患者术中均未发生后囊膜破裂、玻璃体脱出,术后未发生视网膜脱离、角膜失代偿等严重并发症。眼内压2组术后较术前均显著降低(P<0.05),观察组较对照组下降更为显著(P<0.05);最佳矫正视力观察组术后较术前及对照组术后均显著升高(P<0.05),对照组术后较术前有升高,但差异无统计学意义(P>0.05)。结论超声乳化白内障吸除术联合人工晶体植入可明显改善闭角型青光眼患者的视力,降低眼内压;疗效确切,安全可靠。

Clinical Effect of Phacoemulsification Combined with Intraocular Lens Implantation on Angle-Closure Glaucoma

Objective To observe the clinical effect of phacoemulsification combined with intraocular lens implantation on angle-closure glaucoma. Methods Sixty-eight patients with angle-closure glaucoma were randomly assigned to receive either compound trabeculectomy （control group, n=34） or phacoemulsification combined with intraocular lens implantation （observation group, n=34）. The best corrected visual acuity and intraocular pressure were compared between the two groups before and 3 months after operation. In addition,operative complications were observed in both groups. Results No intraoperative posterior capsular rupture and vitreous prolapse and postoperative retinal detachment and corneal decompensation occurred in both groups. After operation, intraocular pressure decreased in both groups,and the decrease in observation was more obvious than that in control group（P〈0.05）. Moreover, the best corrected visual acuity increased after operation. （However, no significant difference in the best corrected visual acuity was found between the two groups （P〈0.05）. Conclusion Phacoemulsification combined with intraocular lens implantation is effective, safe and reliable for improving visual acuity and reducing intraocular pressure in patients with angle-closure glaucoma.
