新型弹簧圈栓塞颅内动脉瘤临床分析

摘    要:目的分析水凝胶弹簧圈及复杂形弹簧圈栓塞治疗颅内动脉瘤的安全性和致密程度。方法水凝胶弹簧圈及复杂形弹簧圈栓塞治疗颅内动脉瘤16例,首先用复杂形弹簧圈成篮,然后用水凝胶弹簧圈进行篮内填塞,最后再用裸圈进行瘤体及瘤颈的致密填塞。结果13个动脉瘤获得致密填塞,3个动脉瘤有瘤颈残留。栓塞密度38%~90%(平均63%)。随访时间2个月~6个月,未发生破裂出血或脑缺血事件。结论水凝胶弹簧圈及复杂形弹簧圈栓塞作为新一代弹簧圈其在颅内动脉瘤治疗中的应用是安全、有效的,可显著提高栓塞的致密程度,长期确切的疗效有待于进一步的经验累及和随访。

Abstract:Objective To essay the safety and density of expandable hydrocoil and Microplex-3D coils for intracranial aneurysms. Methods Sixteen Patients with intracranial aneurysm were treated by hydrocoil and Microplex-3D coils. Microplex-3D coils were firstly used to form a supportive basket, then hydrocoil were introduced for further intrabasket filling and lastly detachable bare platinum coils were placed again to complete the dense packing and covering of the aneurysm neck. Results Complete occlusion was achieved in 13 aneurysms and neck remnant in 3 aneurysms. The embolic material/aneurysm volume ratio ranged from 38-90 %, with an average of 63%. There were no bleeding or cerebral ischemic attack for all patientsundergoing clinical follow up for 2 to 6 months after the procedure. Conclusion To treated intracranial aneurysm with hydrocoil and Microplex-3D coils is safe and effective and can also increase the packing density. But the limited follow up does not allow for any conclusion regarding the long term outcomes.
