系统性红斑狼疮患者血清apo M 的表达及其与疾病活动度的相关性

摘    要:目的:研究系统性红斑狼疮(systemic lupus erythematosus,SLE)患者血清载脂蛋白M(apolipoprotein M,apoM)的含量及其与病情活动度的相关性。方法:选取2013年4月至11月来中南大学湘雅二医院就诊的SLE患者65例(SLE组)和对照组120例,根据SLE疾病活动度指数(systemic lupus erythematosus disease activity index,SLEDAI)将SLE组又分为SLE轻度活动组、中度活动组、重度活动组(分别n=16,16,33),对照组又分为疾病对照组(n=60)和健康对照组(n=60)。采用酶联免疫吸附法(enzyme-linked immunosorbent assay,ELISA)测量SLE组和对照组血清apo M含量,并同时检测两组人群血清总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL)、载脂蛋白A1(apo Al)、低密度脂蛋白(LDL)、载脂蛋白B(apo B)和抗dsDNA抗体,观察SLEDAI和抗dsDNA抗体同apo M的相关性。结果:和健康对照组相比,SLE组的apo M和HDL显著降低,抗dsDNA抗体、TG和apo B均显著升高,差异均有统计学意义(均P<0.05);SLE轻度、中度、重度活动组三组相比,apo M血清水平呈下降趋势,但差异无统计学意义(均P>0.05);SLE患者血清apo M水平与SLEDAI和抗dsDNA抗体表达呈显著负相关(分别r=−0.551,−0.562,均P<0.01)。结论:SLE患者的血清apo M含量相比于健康对照组显著下降,且与SLE疾病活动度有关,可作为监测SLE疾病活动度的一种指标。

Association of serum level of apolipoprotein M with disease activity in systemic lupus erythematosus

Abstract:Objective: To explore the association of serum level of apolipoprotein M with disease activity insystemic lupus erythematosus (SLE).Methods: A total of 65 patients with SLE, who came to Second Xiangya Hospital for treatmentfrom April to November in 2013 (SLE group) and 120 age-and sex-matched controls (controlgroup) were studied. Th e SLE group was further divided into three groups according to systemiclupus erythematosus disease activity index (SLEDAI): a mild activity group, a moderate activitygroup and a severe activity group (n=16, 16, 33, respectively). The control group was also dividedinto a disease control group (n=60) and a healthy control group (n=60). The serum levels of apoM were measured by enzyme-linked immunosorbent assay (ELISA). Other indicators includingTC, TG, HDL, LDL, apo A1, apo B, and anti-dsDNA antibody were detected. The correlationbetween SLEDAI or anti-dsDNA antibody and apo M was assessed.Results: Compared with the healthy control group, the expression levels of apo M and HDL weredecreased significantly (both P<0.05), and the expression levels of anti-dsDNA antibody, TG, apo Bwere increased significantly in the SLE group (all P<0.05). Comparison among the three subgroups,no significant differences in apo M were found (all P>0.05). The serum concentration of apo M wassignificant negatively correlated with SLEDAI and anti-dsDNA antibody (r=−0.551, −0.562, bothP<0.01).Conclusion: Compared with the healthy control group, the serum levels of apo M in patients withSLE are significantly decreased. The apo M is closely correlated with disease activity of SLE and itmight be used as an indicator to monitor the disease activity of SLE.
