普罗帕酮和胺碘酮转复胸部手术后并发阵发性心房颤动疗效比较

目的比较普罗帕酮和胺碘酮转复胸部手术后并发阵发性心房颤动的疗效及安全性。方法选择胸部手术后2天内发作的心房颤动患者42例,分为普罗帕酮组和胺碘酮组各21例,分别静脉予普罗帕酮70~140m g与胺碘酮150~300m g,效果不佳者静滴维持,观察8小时内的转复率及安全性。结果普罗帕酮组和胺碘酮组2小时内的转复率分别为76.2和19.0%,8小时内的转复率分别为95.5和71.4%,两组2、8小时转复率比较均有显著性差异(P<0.05)。用药后均未出现严重不良反应。结论普罗帕酮转复胸部手术后并发阵发性心房颤动的成功率较胺碘酮高。

Effects of propafenone and amiodarone on paroxysmal atrial fibrillation after thoracic operation:A comparative study

Objective To compare the efficacy of propafenone withamiodarone on paroxysmal atrial fibrillation after thoracic operation.Methods Forty-two patients with paroxysmal atrial fibrillation after thoracic operation were randomly divided into two groups: propafenone(n=21) and amiodarone(n=21) groups.Propafenone(70-140mg) or amiodarone(150-300mg) was injected intravenously.Results The conversion rate to sinus rhythm was 76.2%(16/21) by propafenone and 19.0%(4/21) by amiodarone during 2 hours.(P<0.05).The total efficacy in 8 hours was 95.5%(20/21) in the propafenone group and 71.4%(16/21) in the amiodarone group(P<0.05).There were no remarkable adverse effects in the two groups.Conclusion It is suggested that intravenous propafenone is more effective and safe than amiodarone for rapid conversion of paroxysmal atrial fibrillation to sinus rhythm in patients without cardiac failure and hypotension.
