周围型肺癌CT征象与病理对照研究

目的探讨原发性周围型肺癌(以下简称肺癌)螺旋CT征象与组织病理学间的相关性。方法回顾性分析56例原发性周围型肺癌螺旋CT征象及组织病理类型,对各种CT征象及其组织病理类型进行统计对照。结果不同组织病理类型病灶大小无统计学差异(P>0.05)。鳞癌病灶的分叶征较其他组织学类型病灶多见(P<0.05),且多伴有肺炎;腺癌病灶的毛刺征、胸膜凹陷征较其他组织学类型病灶多见(P<0.05),且多伴有胸腔积液;肺泡癌病灶的空泡征较其他组织学类型病灶多见。不同病理类型病灶的平扫CT值,增强60s后CT值及CT差值均无统计学差异(P>0.05)。结论原发性周围型肺癌螺旋CT征象与组织病理类型之间存在着一定的内在关系,CT检查可为周围型肺癌初步定性。

CT Appearances of Peripheral Pulmonary Carcinoma :A Comparison with Histopathological Findings

Objective To study the correlation between histopathological findings and spiral CT appearance of peripheral pulmonary carcinoma.Methods Spiral CT findings and histopathological records of peripheral pulmonary carcinoma in 56 cases were analysed retrospectively,and their relation was comparatively analyzed.Results The diameter of lesions were no difference in all kinds of histopathological group(P>0.05).The lobular sign was markedly more in peripheral pulmonary squamous carcinoma than in other histopathological group(P<0.05) and often with pneumonia;the spicule sign and pleural indentation were obviously more seen in peripheral pulmonary adenocarcinoma than in others (P<0.05) and often with pleural effussion ;the vacuole sign was markedly more in peripheral pulmonary alveolar cell carcinoma than that in others (P<0.05) .There was no difference in all kinds of histopathological group in CT value of plain scan and enhancement scanning (P>0.05).Conclusion There are certain relation between spiral CT appearance and histopathology in peripheral pulmonary carcinoma.CT appearance may be one of the evidence in diagnosing the histopathological type in peripheral pulmonary carcinoma.
