肺炎克雷伯菌多药耐药株的耐药机制研究

摘    要:目的 调查1株多药耐药肺炎克雷伯菌(MDRKP)58种耐药相关基因与可移动遗传元件的存在情况. 方法 对分离自我国内地的1株MDRKP,采用聚合酶链反应(PCR)及序列分析的方法分析58种β-内酰胺类、氨基糖苷类,喹诺刚类和其他广谱抗菌药物耐药相关基因与可移动遗传元件. 结果 该株MDRKP检出TEM-1、SHV-11(均测序比对证实)、tetA、mafA、trbC、tnp513、ISEcp1基因,且gyrA基因存在突变,其余50种基因未检出. 结论 该菌株耐Ⅰ~Ⅲ代头孢菌素抗菌药物,可能与同时产TEM-1型和SHV-11型β-内酰胺酶引起牵制耐药机制有关,该株MDRKP表型为多药耐药与gyrA基因突变及获得可移动遗传元件和多种耐药基因相关,在肺炎克雷伯菌中检出ISEcp1为国内首次报道.
