| 摘 要： | 本文分析了东西方学者对胃癌外科治疗的分歧与近年逐渐取得共识的趋向;介绍了早期胃癌与进展期胃癌合理外科治疗的原则;近年主张早期胃癌不必切除大网膜与网膜囊.进一步确定了中、上部胃癌胰脾区清除范围,行保存一线,切除动脉干及脾的临床价值.

| Abstract: | There are differences of surgical treatment of gastric cancer in Eastern and Western countries,but recent common understandings are elucidated in this paper,some principles how to choose rational surgical treatment of early and advanced gastric cancer are also recommended in the article.In recent years,it is advocated that omentum and omental sac need not be excised in early gastric cancer,the indication of pancreas and spleen resection was defined for tumors located in the middle and upper third of the stomach,the clinical significance of preserving pancreas and excising spleen artery and spleen was evaluated in advanced gastric cancer. |
