内镜联合手术对老年胆囊结石合并胆总管结石的疗效分析

目的探讨内镜下联合手术对于老年胆囊结石并胆总管结石的疗效。方法将62例老年胆囊结石合并胆总管结石患者随机分为内镜下乳头括约肌切开取石联合腹腔镜胆囊切除术(EST+LC)组与胆总管探查取石术(LCBDE)组,各31例,分析这两种微创方法的疗效及住院时间、手术时间、术后并发症、术后恢复等情况。结果两组在手术时间、住院时间(不留T管)、肠鸣音恢复时间、术后并发症方面比较差异有统计学意义(P<0.05),术后恢复较为理想。结论对于老年胆囊结石并胆总管结石症状明确的患者,采用LCBDE术式,疗效肯定,安全、有效,值得临床上进一步研究和推广。

Therapeutic Effect of Combined Endoscopic Surgery on the Senile Cholecystolithiasis and Choledocholithiasis

Objective To discuss the therapeutic effect of combined endoscopic surgery for senile cholecystolithiasis and choledocholithiasis.Methods 62 cases of senile cholecystolithiasis and choledocholithiasis were randomly divided into two groups,31 cases in each:endoscopic sphincterotomy combined laparoscopic cholecystectomy(EST+LC)group and lithotomy of common bile duct exploration(LCBDE)group.Therapeutic effect,hospital stay,surgery time,postoperative complications,postoperative recovery etc.of the two minimally invasive methods were analyzed.Results LCBDE group was better than those of EST+LC group(P<0.05)in the operation time,hospital stay(without T tube),borborygmus recovery time,and postoperative complications,with ideal postoperative recovery.Conclusion For elderly patients with cholecystolithiasis and choledocholithiasis with clear symptoms,LCBDE is safe,effective,and worthy of further clinical research and popularization.
