针灸治疗原发性痛经患者血液流变学效应

目的探讨针灸治疗原发性痛经患者血液流变学效应。方法原发性痛经患者38例，年龄18～24岁，于月经前3d针刺三阴交、血海，经期针刺次露、承山两穴，针后加灸，治疗3d。连续治疗3个月经周期，观察临床疗效，针灸治疗前后检测血液流变学指标。结果38例原发性痛经患者临床总有效率94．7％；血浆黏度、不同切变率下的全血黏度、红细胞压积及红细胞最大聚集指数明显降低，红细胞最大变形能力增强（P〈0．01）；纤维蛋白原虽有不同程度的改变，但无统计学意义。结论原发性痛经的主要病机与血液的高“浓、黏、凝、集”状态有关，针灸具有改善血液流变性，降低血液黏度，改善子宫微循环的作用，从而使疼痛缓解。

Acupuncture Effect on Hemorheological Indexes in Patients with Primary Dysmenorrhea

Objective To investigate the acupuncture effect on hemorheological indexes in patients with primary dysmenorrhea.Methods 38 patients with primary dysmenorrhea,18 - 24 years old,3d before menstruation,were acupunctured sanyinjiao,xuehai,acupuncture and moxibustion treatment 3d on cimiao,chengshan in menstruation.Consecutive treatment of three menstrual cycles to observe the clinical efficacy and hemorheological indexes with acupuncture before and after treatment of primary dysmenorrhea.Results Total clinical effective rate of 38 patients with primary dysmenorrhea is 94.7%;whole blood viscosity, plasma viscosity in different shear rate and maximum aggregate of erythrocyte indices were declined obviously, maximum deformation of erythrocyte indices were significantly increased（P 〈 0.01）;there were varying degrees of change in fibrinogen without statistical significance.Conclusion The main pathogenesis of primary dysmenorrhea is related to blood states with high concentration of erythrocyte,high viscosity states,hypercoagulability and high aggregation states.Our results showed acupuncture could improve hemorheological indexes,reduce blood viscosity, improve microcirculation in the uterus to relieve pain.
