高血压病患者血浆心钠素与左房血流动力学变化的关系研究

应用超声心动图技术和放射性免疫测定法对１００例高血压病患者左房血流动力学变化与血浆心钠素水平的关系进行分析研究。结果示：高血压左室肥厚组和非左室肥厚组血浆心钠素水平显著高于正常对照组（Ｐ＜０．００１）；且左室肥厚组与非左室肥厚组之间亦有显著性差异。血浆心钠素水平与左房功能指标ＬＡＶ、ＬＡＴ、ＬＡＰ、ＬＡＦ及ＭＡＰ呈高度线性相关；与ＬＡＤ、ＬＶＭＩ、ＬＶＥＦ、ＰＦＲ、Ａ峰、ＬＶＤ、Ａ／Ｅ呈中度相关（相关系数依次为：０．８１、０．８０、０．７８、０．７６、０．７０、０．６８，－０．６１、０．６０、０．６７、０．５０及０．４８，Ｐ＜０．０１～０．００１），但与Ｅ峰不相关（ｒ＝０．１６，Ｐ＞０．０５）。随着高血压病程及分期的增加，血浆心钠素水平有增加趋势。结论认为定量测定血浆心钠素将有助于评价高血压病患者左房血流动力学及左室功能的异常变化程度。

A Study on the Relationships between Plasma Level of Atrial Natriuretic Factor and Changes of Left Atrial Hemodynamics in Patients with Hypertension

The relationship of atrial plasma natriuretic factor (PAN F) in 100 hypertension patients and 50 normal ones for control were studied by echocardiography and radioimmunoassay.PANF of all these 100 patients either with (36) or without (64) left ventricular hypertrophy showed higher PANF level than those normals (P<0.001 ).However, significant PANF differences were found between the former two diseased groups. PANF was found to be highly correlated to the left atrial indexes, including left atrial volume (LAV), left atrial tension (LAT), left atrial pressure (LAP),left atrial ejection force (LAF) and mean arterial pressure (MAP), (r=0. 81, 0. 80, 0. 78, 0. 76, 0.70 respectively, P<0.001); moderately linear correlated to left atrial diameter (LAD), left ventricular mass index (LVMI), left ventricular ejction fraction (LVEF),peak filling rate (PFR), peak velocity A, left ventricular diameter (LVD) and A/E ratio(r=0. 68, 0. 64, -0. 61, -0. 60, 0. 57, 0. 50, 0. 48 respectively. P<0. 001-0.01), but not correlated to peak velocity E (r=0. 16, P>0.0).That the more the hypertension troubled, the more the PANF elevated makes an useful indicator for hypertension Prognosis.
