多普勒超声心动图检查对缺血性心肌病左室重构的研究

本文应用多普勒超声心动图对５１例缺血性心肌病（ＩＣＭ）患者的左室重构进行研究。结果显示。ＩＣＭ组的各项指标与正常组比较均有显著差异（Ｐ＜０．０１）。ＩＣＭ组的ＬＶＤｄ、ＬＶＤｓ、ＰＣＷＰ和ＥＳＳ显著增大，ＳＶ、ＣＩ、ＥＦ、ＭＶＣＦ明显降低。结论，ＩＣＭ的左室重构与缺血性心肌损伤（包括冠脉闭塞和冠脉狭窄）引起的左室扩张、肺毛细血管楔压和室壁应力增加有关。多普勒超声心动图对ＩＣＭ左室重构的研究具有重要临床价值。

Study of Left Ventricular Remodeling in Patient with Ischemic Cardiomyopathy by Doppler Echocardiography

Left ventricular remodeling was studied for 5 1 patients with ischemic cardiomyopathy (ICM)and 20 normal control by Doppler echocardiography. The results showed that parameters of Doppler e chocardiography in ICM group were significantly diffenerent in comparison with those in normal control group(P<0.01). The left ventricular end-diastolic diameters, left ventricular end-systolic diameters, pulmonry capil lary wedge pressure(PCWP)endsystolic stress(ESS) were all increased, but stroke volume, cardiac index, e jection fraction and mean velocity of curtail fibers(MVCF) were significantly decreased in ICM group. It is con cluded that left ventricular remodeling in ICM was related to left ventricular dilatation, increased PCWP and ESS which caused by ischemic myocaniial injury included coronary artery occlusion and narrowing.
