电视胸腔镜手术与传统外科开胸术治疗外伤后血胸的对比研究

目的对比电视胸腔镜手术与传统外科开胸手术治疗外伤后血胸的优劣势,总结应用其应用价值。方法回顾性分析笔者所在医院胸外科自2009年1月～2011年12月收治的外伤性血胸患者80例,其中40例行电视胸腔镜手术治疗,列为观察组;40例行传统开胸手术治疗,列为对照组,对比两组患者平均手术时间、术中出血量、术后引流量、引流管留置时间、术后疼痛时间及住院时间。结果观察组与对照组比较平均手术时间、术中出血量、术后引流量、术后疼痛时间、引流管留置时间及住院时间,差异均有统计学意义(P<0.05),观察组具有显著优势。结论电视胸腔镜手术手术时间、引流管留置时间及住院时间短,术中出血量、术后引流量少,胸壁创伤轻微,较传统开胸手术具有更好的治疗优势,值得在临床工作中积极地推广应用。

Comparative study of video-assisted thoracoscopic surgery and conventional surgical thoracotomy in treatment of post-traumatic hemothorax

Objective To compare the advantages and disadvantages of video-assisted thoracoscopic surgery and conventional surgical thoracotomy in the treatment of post-traumatic hemothorax,and summarize its application value.Methods Eighty patients with post-traumatic hemothorax admitted to and treated in our hospital from January 2009 to December 2011 were analyzed retrospectively.Forty patients receiving video-assisted thoracoscopic surgery served as the observation group while 40 patients receiving conventional surgical thoracotomy served as the control group.The average operative time,intraoperative blood loss,postoperative drainage amount,indwelling time of drainage tube,time of postoperative pain and length of stay of the two groups were compared.Results The observation group and the control group were significantly different in the average operative time,intraoperative blood loss,postoperative drainage amount,indwelling time of drainage tube,time of postoperative pain and length of stay (P < 0.05);The observation group had significant advantages over the control group.Conclusion The video-assisted thoracoscopic surgery has shorter operative time,indwelling time of drainage tube and length of hospital stay,less intraoperative blood loss and postoperative drainage amount,slight chest wall trauma,and better treatment advantages over the conventional surgical thoracotomy,thereby worthy of clinical promotion and application.
