前列腺增生合并膀胱结石同期手术治疗的疗效分析

目的研究前列腺增生合并膀胱结石同期手术治疗的疗效。方法对笔者所在医院2008年6月～2011年6月收治的80例同期手术的前列腺增生合并膀胱结石的患者,电切镜下采用电切环钩出结石、采用气压弹道碎石和小切口膀胱切开取石,后采用经尿道前列腺电切术治疗前列腺增生合并膀胱结石。结果 76例患者治疗效果良好无并发症,治疗总有效率为95%,患者手术前后IPSS、QOL、Qmax3种参数比较差异有统计学意义(P<0.05)。结论直径<1.0cm的结石,使用电切环直接夹取结石,如果不成功再用输尿管镜经电切镜外鞘气压弹道碎石术;对于1.0～2.5cm结石,尽量采用气压弹道碎石术处理;如果直径>2.5cm结石,则采用耻骨上小切口膀胱切口取石术或者钬激光碎膀胱结石。

Efficacy analysis of prostatic hyperplasia with bladder stones in the surgical treatment for the same period

Objective To analyze the clinical efficacy of prostatic hyperplasia with bladder stones in the surgical treatment for the same period.Methods 80 cases with prostatic hyperplasia with bladder stones in our hospital from June 2008 to June 2011 were selected.Stones were hooked out by resectoscope transurethral ring then used pneumatic lithotripsy,got stone by cutting bladder small incisions,used transurethral resection to treat prostatic hyperplasia with bladder stones.Results 76 cases had well treatment efficacy and had no complications.The total efficacy was 95%.The differences were significant between IPSS,QOL,Qmax three parameters before and after surgery (P < 0.05),and it had statistically significant.Conclusion The stones' diameter is less than 1.0 cm and it should use transurethral ring directly grip stones.If it's not successful,use ureteroscopic resectoscope sheath pneumatic ballistic lithotripsy.The stones' diameter is between 1.0 and 2.5 cm and it's better to use pneumatic lithotripsy.The stones' diameter is more than 2.5 cm,use suprapubic bladder incision small incision lithotomy or holmium laser crushed bladder stones.
