尼索地平对实验性心肌梗塞的作用

摘    要:应用两种实验性心肌缺血模型,研究二氢吡啶类钙拮抗剂──尼索地平的抗心肌缺血作用。结果表明:家兔静注尼索地平可明显改善团结扎兔冠脉前降支所引起的心肌缺血和心肌梗塞状况。结扎后24,48h,尼索地平(2、5μg·kg-1)能显著缩小梗塞区,减小心肌梗塞范围,减少ECG的∑ST段位移幅度。硝苯吡啶(40μg·kg-1)组也可显著缩小心肌梗塞范围并减少∑ST段位移幅度,其作用效果与尼索地平5μg·kg-1相似。大鼠口服尼索地平对异丙肾上腺素所致的心肌缺血状况改善并不明显。尼索地平(1、3、9mg·kg-1)均不能显著减少∑ST段位移幅度和心脏病理分级,与溶剂对照组(10g·L-1CMC)比差异无显著性(P>0.05)。

Abstract:In rabbits, the left anterior descending coronary artery was ligated for 48 h. Nisoldipine (iv 2μg·kg-1,5μg·kg-1) was given 3~5 min before ligation and 24, 48 h after ligation. Nisoldipine decreased myocardial infarctive size (MIS) from 24.2 % ±4.5% to 13.3%±4.2% ( P< 0.01)and 10.4% ±5.4% (P<0.01), respectively;∑ ST segment elevation was improved from 2.57±0.40 mV to 1.29±0.65 mV (P<0.01) and 0.84±0.37 mV (P< 0.01), respectively at 24 h and from 1.98±0.37 mV to 1.12±0.55 mV (P<0.05) and 0.63±0.65 mV (P<0.05), respectively at 48 h.At the meantime, Nifedipine(40μg ·kg-1) also decreased the MIS and improved ∑ST segment elevation. However, in rats, Nisoldipine (ig 1 mg·kg-1 ,3 mg·kg-1· 9 mg·kg-1) had no effect on ∑ ST segment elevation and pathological grades of hearts with Isoprenaline-induced myocardial nerosis.
