睾丸扭转19例报告并文献复习

目的探讨睾丸扭转的诊断与治疗方法,提高其治疗水平。方法对本院收治的19例睾丸扭转的临床表现、诊断及治疗进行回顾性分析。结果发病6h内的8例行睾丸复位固定术,睾丸均存活,发病时间超过6h的11例中,除2例外,其余均行患侧睾丸切除术。对所有患者的健侧睾丸进行探查固定。结论彩色多普勒血流动态显像(CDFI)是首选的辅助检查手段,对明确诊断十分重要;早期行阴囊内探查是提高睾丸存活率的关键。

Testicular Torsion(report of 19 cases and review of literature)

Objective To investigate the diagnosis and management of testiculartorsion,and improve the outcome of its treatment. Methods Clinical data of 19 cases of testicular torsion were retrospectively analyzed. All the 19 cases had the essential clinical symptom of colic in testis and/or abdominal.12 cases had the positive Prehn's sign and horizontal lie to the testis had been found in 4 cases. Color doppler flow imaging (CDFI) showed that the testicular blood flow in 17 cases was reduced or disappeared. Results 8 cases who received scrotal ex-ploration within 6h from attack time had testicular survival after detorsion and orchidopexy. Among 11 cases who received operation more than 6h from attack time only 2 had testicular survival. The contralateral testicles were performed preventive orchiopexy. Conclusion The misdiagnosis rate of testicular torsion in the primary diag-nosis is high. CDFI is helpful for the diagnosis of early testicular torsion. Early diagnosis and emergency scrotal exploration are keys for testicular survival.
