数字钼靶X线摄影对早期乳腺癌诊断的应用研究

目的评价早期乳腺癌的数字钼靶x线摄影表现。方法收集我院自2003年10月至2013年3月共10150名乳腺检查及体检者，回顾性分析经手术病理证实的56例早期乳腺癌的数字钼靶x线片，分析其临床表现、年龄特征、病理类型及x线表现特征。结果①无临床表现者49例，发病年龄27～76岁，其中40～49岁年龄段者占总病例数的42％，X线表现钙化27例（其中18例无肿块），小结节19例（34％）。非对称性局限性密度增高影伴结构扭曲或紊乱10例（18％）；未扪及肿块者22例（39％）；导管原位癌（DCIS）及导管原位癌伴早期浸润37例（66％），浸润性导管癌17例（30％），浸润性小叶癌2例（4％）。②乳腺数字钼靶X线可显示早期乳腺癌病变微细结构。结论数字钼靶x线成像可以更清晰地显示乳腺病变的特征，对早期乳腺癌的诊断具有重要意义和价值。

The applied research of digital mammography in the diagnosis of early breast cancer

Objective To evaluate the symptoms of the digital mammography in the early breast cancer. Methods Ten thousand and one hundred fifty cases from those who had breast examinations or physical examina- tions were collected in our hospital. Fifty-six cases of the early breast cancer were confirmed by the operation and the pathology analysis, in which the clinical manifestations, the characteristics of the age, the pathological type and the X- ray features were analyzed. Results Forty-nine patients without clinical symptoms, whose pathogenic age ranged from 27 to 76 years old, among them 40-49 years old accounted for 49% , and in the X-ray films the calcification showed in 27 cases （including 18 cases without bumps）, small nodules showed in 19 cases（34%）. The asymmetrical localized high density image with structural distortions or disorder could be seen in 10 cases; 22 cases were non-palpable（39%）; 37 cases were duct carcinoma in situ （DCIS） and DCIS with early infiltration（66%）; 17 cases were infiltrating duct car- cinoma（30%）; 2 cases were invasive lobular carcinoma（4%）. The breast digital mammography can show the fine struc- ture in the early breast cancer lesions. Conclusion Digital mammography imaging can clearly display the fea-ture of breast lesions, and has important significance and value in the diagnosis of the early breast cancer.
