食管癌术后呼吸衰竭的治疗

分析１０例食管癌、贲门癌术后呼吸衰竭的发病和治疗。发病率占同期食管癌、贲门癌手术６．１％（１０／１６５）。治愈６例，死亡４例，死亡例数占同期食管癌贲门癌手术病人数的２．４％。术后呼吸衰竭治疗的关键在于尽早使用辅助通气，彻底清除呼吸道内分泌物，控制肺内感染，早期发现并处理胸内其它病变。单纯低肺功能也非绝对手术禁忌症。对食管癌贲门癌术后呼吸衰竭，预防比治疗更重要。

THE TREATMENT OF PATIENTS WITH RESPIRATORY FAILURE AFTER OPERATION OF ESOPHAGEAL AND CANDIAC CANCER

The incidence and treatment of respiratory failure after operating esophagealand cardiac carcinoma were analized.There were ten patients, the incidence rate was 6. 1%(10/165),the death rate was 2.4%(4/165),slx patients were cured(3.6%).The key pointsof treatment of respiratory failure after operation were as follow:assisted ventilation assoon as possible,evacuation of respiratory secretion as clear as we can,control of pul-monary infection,early discovery and management of other intrathoracic pathology.Thesimple low respiratory function is not the definite contraindication of operation,Prophy-laxis of respiratory failure after esophageal and cardiac carcinoma operation is more impor-tant than therapy.
