摘    要:目的探讨红细胞输注联合高压氧治疗一氧化碳中毒后迟发性脑病(DEACMP)的疗效。方法对77例中、重度一氧化碳中毒后发生DEACMP患者随机分为治疗组和对照组,治疗组35例,施行红细胞输注、高压氧和内科基础治疗。对照组42例,施行高压氧和内科基础治疗。治疗2周、2个月后进行NIH-SS、Bathel指数评分,并观察Hb、Hct、脑电图、脑MRI变化。结果治疗2周和2个月后,治疗组NIHSS、Bathel指数评分及Hb和Hct的改善均优于对照组(P<0.05)。治疗2个月后,脑电图和颅MRI好转率明显高于对照组,差异均有统计学意义(P<0.05)。结论红细胞输注联合高压氧治疗DEACMP疗效显著。

Combination erythrocyte transfusion and hyperbaric oxygen treatment in delayed encephalopathy after carbon monoxide poisoning

Abstract:Objective To study the clinical effects of integrated erythrocyte transfusion and hyperbaric oxygen treatment in delayed encephalopathy after acute carbon monoxide poisoning (DEACMP). Methods Seventy-seven cases with DEACMP were divided into control group (42 cases) and treatment group (35 cases) randomly. In treatment group, the integrated erythrocyte transfusion and hyperbaric oxygen treatment with medicine therapy were performed. The cases in control group were underwent hyperbaric oxygen and medicine therapy. At the time point 2 weeks and 2 months,the NIHSS scoring, Bathel index scoring, Hb, Hct, electroencephalography and brain MRI were observed and compared. Results There was significant improvement in treatment group than control group, inclu- ding NIHSS scoring, Bathel index, Hb and Hct after two weeks and two months treatment ( P 〈 0.05 ). The progress of EEG and brain MRI was obvious in treatment group than control group at time point 2months ( P 〈0.05 ). Conclusion There was reliable clinical effect of integrating the erythrocyte transfusion and hyperbaric oxygen treatment in DEACMP.
