C-myc、P16和Bcl-2蛋白在胆管癌中表达及意义

摘    要:目的检测胆管癌组织中C—myc、P16、Bcl-2蛋白表达,探讨其与胆管癌病理的关系及其在胆管癌发生、发展中的作用。方法用免疫组化法检测胆管癌、癌旁组织、胆管囊状扩张症组织及医源性胆道损伤的胆管组织中C—myc、P16和Bcl-2蛋白表达。结果C—myc、P16在胆管癌组织中的阳性表达率明显高于其他组(P〈0.05);C—myc、P16阳性率在高、中分化腺癌中明显高于低分化腺癌(P〈0.05);C-myc、P16阳性率在Ⅰ级胆管癌明显高于Ⅱ、Ⅲ级胆管癌(P〈0.05);癌旁组织中Bcl-2阳性表达率为100%,明显高于癌组织(P〈0.05);Bcl-2阳性率在不同组织学级别中差异无显著意义(P〉0.05);无转移组和有转移组组间比较,C—myc阳性表达率差异有显著性(P〈0.05),P16和Bcl-2阳性率差异无显著性(P〉0.05)。结论C—myc、P16蛋白表达特征可反映胆管癌生物学行为和恶性程度,Bcl-2过度表达与胆管癌变过程有关,可能是胆管癌的早期事件。

Abstract:Objective To examine the expression of C-myc,P16,Bcl-2 proteins in hilar cholangiocarcinoma(HCC)in order to study their relation with the pathologic characteristic of HCC and their roles in the development of HCC.Methods Using immunohistochemical method to detect the expression of c-myc,P16,Bcl-2 proteins in HCC tissues,paracancer tissues,cholagioectasis tissues and normal bile duct tissues.Results As to the expression of C-myc and pl6,HCC tissue was significantly higher than others(P <0.05),well and medial differentiated HCC tissue obviously higher than poor ones(P <0.05),class I HCC significantly higher than class II or III HCC(P <0.05).As to the expression of Bcl-2,paracancer tissue(100%)was significantly higher than other 3 tissues,but differentiation degree of HCC shows no obvious difference;When compared between metastasis and nonmetastasis group(P <0.05),it is the expression of C-myc other than pl6 and Bcl-2 shows a statistic difference(P <0.05).Conclusions The expression of C-myc and P16 proteins probably reflects the biological behavior and malignant degree of HCC,and the over expression of Bcl-2 is related to the development of cholangiocarcinoma.
