Effects of revascularization on left ventricular remodeling and function in patients with acute anterior myocardial infarction

Objective To evaluate the effects of revascularization on left ventricular remodeling,left ventricular systolic and diastolic function in patients with acute anterior myocardial infarction(AAMI).Methods According to the different therapies,forty-four patients with AAMI were divided into 3 groups which were conventional therapy group(n=17),emergency revascularization group(n=11) and selective revascularization group(n=16).The left ventricular end-diastolic dimension(LVEDD),left ventricular end-systolic dimension(LVESD),left ventricular ejection fraction(LVEF),and ratio of early mitral inflow velocity(VE) and late inflow velocity(VA) were measured and calculated by echocardiography on the 1st week,the 3rd month,6th month and 12th month after AAMI.Results Compared with conventional therapy group,LVESD on the 6th month after AAMI in the emergency revascularization group and on the 3rd month and 6th month in the selective revascularization group,as well as LVEDD on the 6th month and 12th month in the emergency revascularization group and on the 3rd month,6th month and 12th month in the selective revascularization group,were significantly decreased(P<0.05).LVEF on the 6th month in both of the two revascularization groups and VE/VA on the 6th month in the emergency revascularization group were significantly higher than those at the same time frames in the conventional therapy group(P<0.05).There were no significant differences of LVEDD,LVESD,LVEF and VE/VA on the same time frames between the two revascularization groups,and on each time points during the follow up time among three groups of AAMI(P>0.05).Conclusion Both of emergency and selective revascularization appears to be superior to drug treatment with respect to suppression of left ventricular remodeling and improving left ventricular systolic function in patients with AAMI,but selective revascularization fail to do diastolic function.
