二次剖宫产172例临床分析

目的探讨第1次剖宫产时不同手术方式对二次剖宫产的影响。方法对1998年1月至2005年12月期间二次剖宫产病例进行回顾性分析。其中第1次手术为传统子宫下段剖宫产术115例(组1),新式剖宫产组57例(组2),对两组二次手术腹腔粘连程度、术中出血、手术时间、开腹时间与术后病率及术后排气时间等进行比较。结果新式剖宫产组患者与传统子宫下段剖宫产患者相比,腹腔粘连严重,手术时间与开腹时间长,术中出血量多,术后排气时间长,差异有统计学意义(P<0.05);结论新式剖宫产二次手术腹腔粘连严重,术中出血多,手术后恢复差,应严格掌握剖宫产指征,慎重选择第1次手术方式。

A clinical study of 172 cases of repeated cesarean section

Objective Discussing the impact of the first various cesarean section types to the second cesarean section.Methods The cases of second cesarean scetion from January 1998 to December 2005 were reviewed and analyzed.The traditional surgery for lower uterine cesarean section is 115 cases(group 1),the new type cesarean section is 57 cases(group 2),comparing two abdominal surgery adhesion,bleeding,operative time,abdominal-opening time and postoperative morbidity rate and postoperative exhaust time.Results Cesarean section patients compared with traditional lower uterine segment cesarean section patients,abdominal adhesions is more severe,operative time and obdominal-opening time is longer,the average blood is more and the time of fart after operation is significantly longer.Conclusion Repeated cesarean section surgery of abdominal adhesions and bleeding is more,poor recovery after surgery,the indication for cesarean section should be strictly controlled.Carefully sclecting the first surgery.
