膀胱实性占位性病变的超声造影研究

目的探讨超声造影在膀胱实性占位性病变中的应用价值。方法应用超声造影和对比脉冲序列成像技术对48例二维超声发现膀胱壁局限性增厚、低回声结节、可疑膀胱实性占位性病变的患者行实时超声造影,比较造影剂灌注时相和变化规律,并与病理对照。结果造影提示膀胱恶性肿瘤43例,良性病变5例。病理证实膀胱恶性肿瘤41例,良性病变7例。超声造影诊断膀胱恶性肿瘤的敏感性为100%,特异性为77.78%,准确性为96.00%。80.49%的膀胱恶性肿瘤超声造影表现为快速充填、缓慢廓清,14.63%表现为快速充填、快速廓清,4.88%与膀胱壁同步充填同步廓清。结论超声造影在诊断膀胱实性占位性病变及鉴别膀胱肿瘤的良恶性方面有重要的临床使用价值。

The application of contrast-enhanced ultrasound in the diagnosis of bladder occupied lesions

Objective To explore the value of contrast-enhanced ultrasound in bladder occupied lesions.Methods Forty-eight patients suspected bladder occupied lesions underwent conventional and contrast-enhanced ultrasound examinations by injection of SonoVue via peripheral vein.The perfusion time,intensity and its characteristic of the agent were assessed and the results were compared with that of pathology.Results In the 48 lesions,43 lesions were diagnosed as malignant,5 lesions were benign by contrast-enhanced ultrasound;41 lesions were bladder carcinomas,7 lesions were cystitis glandularis by pathology.The sensitivity,specificity and accuracity of contrast-enhanced ultrasound in the diagnosis of baldder malignant tumor were 100%,77.78%,96.00%,respectively.80.49% bladder carcinoma had the characteristics of contrast agent fast wash-in and slow wash-out,but 14.63% had fast wash-in and fast wash-out,other 4.88% was similar with the bladder wall.Conclusions The contrast-enhanced ultrasound has significant value in the differential diagnosis of the bladder occupied lesions.
