湛江地区老年人胃息肉与幽门螺杆菌感染的关系

目的 探讨本地区老年人胃息肉与幽门螺杆菌(Hp)感染的关系,进一步探讨根除Hp对部分胃息肉的治疗价值.方法 对我院2004年至2009年经胃镜活检及病理诊断的116例胃息肉老年人Hp感染情况进行分析,并与同期胃镜检查为慢性胃窦炎且病理报告静止期的86例对照组老年人Hp感染情况进行比较.结果 老年人胃息肉组Hp感染率为58.6%(68/116),对照组Hp感染率为43.0%(37/86),两组比较差异有统计学意义(P<0.05).59例Hp检查阳性胃炎性及增生性息肉患者经根除Hp治疗,2～6个月后复查胃镜发现40例息肉消失.结论 Hp感染与老年人胃息肉有相关性,说明胃息肉的发生、发展可能与Hp感染有关.故对Hp阳性胃炎性息肉及增生性息肉在行息肉摘除术前可先尝试Hp根除治疗,可使部分息肉消失. Abstract： Objective To explore the relationship between gastric polyps in elderly and helicobacter pyloi(Hp) infection in Zhanjiang. Methods One hundred and sixteen elderly patients with gastric polyps were selected.Gastroscpic and histologic mucosal changes were diagnosed by gastroscope and pathology, and was compared with normal elderly. Results Hp infection rates were 58.6% in gastric polyp group and 43.0% in control group.There was significant difference between the two groups.Fifty-nine cases of Hp inflammatory gastric polyps completed eradication and 40 cases of endoscopic polyps had disappeared after 3 to 6 months.Conclusions There is significant relationship between gastric polyps and Hp infection in elderly.However,the occurrence and development of gastritis polyps and hyperplastic polyps maybe related to Hp infection.Some gastric polyps may disappeared after the eradication of Hp might be tried.
