核增殖抗原和生存素蛋白在胃肠道间质瘤中的表达与意义

目的 探讨核增殖抗原(Ki-67)和生存素(survivin)蛋白在胃肠道间质瘤(GIST)中的表达及临床意义.方法 应用免疫组化SP法和病理图像分析系统检测85例胃肠道间质瘤(GIST)患者中Ki-67和survivin的表达情况,结合临床病理学资料分组比较并进行统计学分析.结果 85例GIST中Ki-67和survivin表达的平均灰度值在良性组、潜在恶性组、恶性组中依次递减, 即它们的阳性表达水平逐渐增高.三组两两比较,各组的平均表达水平差异有统计学意义(P<0.05).而且在41个恶性病例中,有浸润或转移的亚组的Ki-67和survivin表达水平显著高于无浸润转移亚组的表达水平(P<0.05).同时统计分析表明GIST中Ki-67和survivin表达有显著的正相关性(P<0.01).结论 Ki-67和survivin高表达导致肿瘤细胞的增殖旺盛和凋亡抑制是GIST发生恶变以及向外侵袭转移的重要机制之一,两者可能用以协助判断GIST的良恶性以及预测肿瘤转移及预后.Abstract：Objective To investigate the expression of Ki-67 and survivin in gastrointestinal stomal tumors (GIST) and their clinical pathologic significance. Methods The expression of Ki-67 and survivin were detected in 85 cases of GIST by immunohistochemical technique. Futhermore,multi-function precise color-image measure system was used for quantitative analysis.The relationship between expression and clinical pathologic data was analyzed.Results Among 85 GISTs, the expression of Ki-67 and survivin in benign,potentially malignance and malignance group were increased gradually.There was a significant difference among the groups of each gene's expression(P<0.05). There was also significant difference in the expression of Ki-67 and survivin between the metastasis subgroup and the non-metastasis subgroup. There was significant positive correlation between Ki-67 and survivin expression(P<0.05). Conclusions The unbalance of the cell proliferation and apoptosis resulted by the increased expression of Ki-67 and survivin is one of the important mechanism in carcinogenesis process and invasion and metastasis of GIST. The expression of Ki-67 and survivin maybe can serve as an important predicting maker for malignancy degree and metastasis and prognosis of GIST.
